OSHA Severe Injury and Fatality Reporting
In 2015, the Occupational Safety and Health Administration (OSHA) updated the fatality and severe injury reporting rules. The list of severe injuries that must be reported to OSHA expanded.
The list of severe injuries expanded to include:
- All work-related fatalities
- All work-related inpatient hospitalizations of one or more employees (changed from three or more employees)
- All work-related amputations
- All work-related losses of an eye
Employers must report by calling the nearest OSHA office during normal business hours or (800) 321-OSHA (6742) at any time.
Businesses in Iowa, South Dakota, North Dakota and Nebraska can also report this information online.
- Employers must report work-related fatalities within eight hours.
- Inpatient hospitalizations of one or more employees, amputations or loss of an eye must be reported within 24 hours of learning about the incident.
OSHA will act on these reports, and it is likely that the report will trigger an inspection.
